Registration Requirements

• Completed Registration form

• Completed Pick up Policy form

• Copy of the student's Birth Certificate

• Copy of the student's updated shot record

• TB or PPD test for the current year

• Completed medical physical examination form signed by the doctor (due before classes begin). Your child cannot be admitted to school without these requirements, pursuant to Public Health and Social Services Law, and School Policy No. 222

• Entry Permit for Non Resident Alien Students

• $200 Non-refundable registration fee
